On 18th & 19th an International conference on assisted reproductive technology was held in
Drs Nadkarni’s Kishore & Purnima a gold medallist couple, who studied in KEM hospital Mumbai, started their currier in a small town of Killa Pardi, with an aim to give Gold standard, hi tech treatment to the rural population of India, 25 yrs ago leaving the lucrative practice of city. Nadkarni hospital grew in size & shape to become the active rural hospital on highway giving emergency treatments to everyone. Due to the green fingers of Dr. Purnima , who has taken education under Dr. Indira Hinduja & Dr. V N Purandare who are the great experts in Infertility treatment the infertile couples from Rural Killa Pardi, Dharampur, Chikhali, Vapi , Valsad started getting pregnant, This couple then went to Germany, USA, Australasia to take advanced infertility, endoscopy & ultrasound training came back to start their own “ IVF canter” called Nadkarni IVF cenrter which has in last 12 yrs given more than 1000 IVF, ICSI pregnancies to not only rural , urban India but patients from Pakistan, USA, UK, New Zealand, Kenya etc.
At the turn of the century they started a “21st century hospital, multifacility which is the only ISO 9001-2000 hospital of Valsad district catering to population of Vapi, Silvassa & Daman. It is now called MINI Hinduja
On March 2007, Nadkarni’s started yet another multifacility hospital at Surat, called 21st century hospital & test tube baby center, which is taken care by their Gold medallist daughter Dr. Pooja who has in the short span of 1 ½ year given 400 pregnancies to the infertile couples of Surat & surroundings, out of which 130 are with the treatment called test tube baby & ICSI- Intracytoplasmic sperm injection.

On 18th October, Saturday there were 2 live workshops on ART – assisted reproductive technology & IUI -the procedure in which sperms of a male partner are enriched to make his wife pregnant. About 150 delegates attended live ART workshop & 120 delegates attended IUI workshop. The procedures of Live ovum pick up, test tube baby, ICSI, TESA , sperm wash etc were done live at the World class IVF centre of 21st century hospital at Surat & it was shown live by satellite to the delegates in Hotel Lords Plaza. The foreign faculty were felicitated. Thanks giving were done by Dr. Kishore Nadkarni. He thanked all faculty, delegates & phrama companies who made it a successful conference. When asked, Dr. Purnima Nadkarni & Dr. Kishore Nadkarni a surgeon Gynec duo said they are doing successful surrogacy since last 1 yr. Out of 7 cases they have attempted so far 4 are successful & ongoing.
Mr. & Mrs a doctor by profession from South Gujarat has all the successful practise & wealth but was sad as both of them did not have their own child. Mrs Sonawala age 44 (name changed) suffered from Obesity (more than 100KG wt), hypertension & diabetes & was unfit to become pregnant. But she had her ovaries functioning & Mr. Soonawala had normal sperm counts. After undergoing many unsuccessful attempts at many clinics In Gujarat & Mumbai they came to meet Dr. Purnima , who suggested them to have a surrogate baby. Mrs. Snehalata from Nadiad who needed money to build up her house came to 21st century hospital to accept surrogate motherhood. She has conceived in first attempt & now having a twin pregnancy of 3 months.
Mr & Mrs Patel, a bank manager from Surat, wife age 39 having Asherman’s syndrome due to infection of tuberculosis in the uterus who has got her endometrium completely destroyed decided to go for Surrogate child. Mrs. Radha ( mother of 2)who needed money for the higher studies of her elder son has taken surrogate babies of Mrs patel & is now pregnant with high levels of beta hCG. Mrs Pooja feels she will be having twin or a triplet pregnancy. We will have to take her extra care that is what pooja says. I am keeping my fingers crossed, hope she should not have more than 2 babies. There is a lot of happiness as well as concern in the Doctor’s eyes.
We asked more details about surrogacy to Mrs. Nadkarni. Let us see, what is this Surrogacy? A surrogate means a lady who takes embryos of the donor couple in her uterus (where egg & sperms of genetic parent are fertilized outside the body in the laboratory by doctor) & replaced in the uterus of a healthy woman. She once conceived (50% chance in 1st attempt) takes the pregnancy to 9 months, delivers the baby & gives it back to the genetic parents. The genetic parents take care of her in all 9 months & 1 ½ months after delivery. Her nutrition , mental & physical health is taken care. Her children & family is also provided support & she is reimbursed for her services. According to doctor Purnima this is the biggest donation – gift of life by a Surrogate mother to an infertile couple & it can’t be matched with any sacrifices or gifts.
Who needs Surrogate mother?
A woman who has absent uterus from birth, who has bad disease in the uterus making it incapable of becoming pregnant like endometrial TB or post abortal, post partum sepsis. Asherman’s syndrome is the one in which the lining of the uterus is destroyed & hence there are adhesions inside endometrial cavity. Sometimes patient has unexplained infertility or recurrent abortions & pregnancy does not go to full term. In such cases the couple may have to hire a womb of a healthy mother.
How is surrogacy done?
The genetic mother is given hormonal injections to get superovulated. The eggs are retrieved with trans vaginal sonography & a long needle. The eggs are harvested in the incubator. Sperms are collected from the genetic father & fertilization of eggs achieved with IVF method or ICSI method ( Intra cytoplasmic sperm injection). The embryos thus formed are replaced in the recipient surrogate mother whose uterus is prepared with Estrogen & progesterone hormones. There is a 50% chance of pregnancy per cycle as she is healthy & fertile woman.
Dr Purnima Nadkarni shres her beautiful experience with a couple from Silvassa. This couple had failed 6 attempts of undergoing IVF in Mumbai & 2 attempts in Killa Pardi. They decided to attempt surrogate mother. Her elder sister volunteered do give her uterus for this generous job. Dr Purnim says at the time of Embryo transfer there were 5 embryos so Purnima decided to transfer 3 embryos in surrogate & 2 in real genetic mother. To her surprise this time not the surrogate but the real genetic mother became pregnant & has delivered a healthy baby girl one month ago. There were tears of joy in doctor’s eyes while describing this case. A healthy baby is the nature’s biggest gift to human being & science & scientific adnvances has made it possible that in 21st century no mother will be without a baby with Ovum donation, embryo donation, surrogacy , ICSI & IVF- ET, TESA, MESA & sperm donation lastly adoption.

Surat: A miracle baby is what Surat got on Thursday when a 28-year-old woman, who did not have her menses till the age of 20 due to a rare genetic disorder, delivered a baby girl.
The woman gave birth with intracytoplasmic sperm injection of eggs (ICSI) and in-vitro fertilisation (IVF) treatment at the 21st Century Hospital and Test Tube Baby Centre. It is being termed as one of the rarest cases of testicular feminisation syndrome — a genetic disorder that makes XY fetuses insensitive to male hormones. Terming it as a unique case, Dr Pooja Nadkarni Singh of the hospital said, “The genotype of the patient is male with feminine features.”
Meeta Mehta, a resident of Palanpur Patia, is feeling on top of the world having got a child, weighing 3 kg, after eight years of marriage. Three years ago, she had given up hope to conceive after knowing about her genetic disorder. Meeta grew up like a normal girl and married at the age of 20. She had consulted doctors in Ahmedabad and Surat and was put on medication, after which she started having her periods. But the real worry started when she could not conceive.
She decided to go for Karyotyping test, which helps find genetic problems, in 2006. The test report came as a major shock as it showed her genotype as male. The related medical jargon simply meant that she was sterile and could not bear children. In a layman’s language, women with this genetic disorder are born looking externally like normal girls, but they are genetically male with short blindpouch vagina and no uterus, fallopian tubes or ovaries.
In 2008, Meeta consulted Dr Pooja and Dr Poornima Nadkarni. After detailed investigation, it was found that she had a rudimentary uterus and that it was possible to give her a child through ICSI and IVF treatment.
